Liability Waiver Form

(Read carefully before signing)

I, {name}, am in good physical condition and fully able to participate in martial arts, self-defense and or contact sports. I am fully aware of the risks and hazards connected with the participation in this activity, including physical injury or even death, and herby elect to voluntarily participate in said activity, knowing that the associated physical activity may be hazardous to me and my property. I voluntarily assume full responsibility for any risks or loss, property damage, personal injury, or death, that may be sustained by me, or loss or damage to property owned by me, as a result of participation in this instruction, or during transit to and from the class/facility.

I hereby release, waive, discharge, and covenant not to sue, Donald Forbes (registered agent), Elite Krav Maga LLC., their officers, managing members, managers, servants, agents, instructors, independent contractors, volunteers, and employees (hereinafter referred to as RELEASEES) for any and all liability, claims, demands, actions and causes of actions whatsoever arising out of or related to any loss, damage, injury, or death, that may be sustained by me, or to any property belonging to me, while participating in physical activity, or while on or upon the premises where the event or instruction is being conducted.

It is my expressed intent that this release and hold harmless agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representative, if I am deceased, and shall be deemed as a RELEASE, WAIVER, DISCHARGE, and CONVENANT NOT TO SUE the above named RELEASEES. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be constructed in accordance with the laws of the State of Florida. If any provision of this document shall be held to be invalid or unenforceable for any reason, the remaining provisions shall continue to be valid and enforceable. If a court finds that any provision of this document is invalid or unenforceable, but that by limiting such provision it would become valid and enforceable, then such provision shall be deemed to be written, construed, and enforced as so limited by the courts.

Participant agrees that any pictures, audio, or visual recordings taken of him/her in connection with the class can be used for publication, promotion, articles, shows, social media, and advertisement without additional consent and without compensation at this time or any other time.

In signing this release, I acknowledge and represent that I have read the forgoing Waiver of Liability and Hold Harmless Agreement, understand it and sign it voluntarily as my own free act and deed: I am not under their influence of any drugs, alcohol, or other intoxicants. I am not suffering from any illness or incapacity. No oral representations, statements or inducements, apart from the foregoing written agreements have been made; and I execute this release for full, adequate and complete consideration fully intending to be bound by same.
